Waiver: In case of emergency, I understand every effort will be made to contact parents/guardian. If I cannot be reached, I hereby authorize the physician selected by the activity leader to hospitalize and secure whatever treatment necessary for my child as named above. I (parent/guardian of the child(ren) on this form) agree to assume and accept all risks arising out of, associated with or related to my or my child's participation in the Activities (whether on or off campus) of the Children's Ministry of Mikado Baptist Church. I agree to waive and release Mikado Baptist Church and/or any members of Mikado Baptist Church who host Activities of the Children's Ministry on his or her property from any and all liability for any loss, damage, injury, or expense that I or my child suffer, or that my next of kin may suffer as a result of my or my child's participation in the activities due to any cause whatsoever.
